Kid's Cottage Child Development Center, LLC

1 of 3 booster seats was expired in one of the transportation vehicles. Note: This was CORRECTED during the inspection as the expired booster seat was removed from the vehicle.

There were no activity plans posted in a toddler room and afterschool rooms. The activity plans posted in the infant room and pre-k room were not current.

Two infants were combined with older children in a toddler room. There was more than an 18 month age difference between the youngest and oldest child in the group. The youngest child was 15 months old and the oldest child was 3 years old.

A caregiver's purse was observed on top of a children's shelf. This posed a safety hazard as the caregiver's purse was within children's reach. Note: This was CORRECTED during the inspection as the purse was stored in an area out of children's reach.

There was more than an 18 month age difference between the youngest and oldest child in a group when an infant was combined with older children. The infant was 14 months of age and the oldest child in the group was 10 years old.

There was no additional employee present in the building during the naptime.

Upon arrival to the operation a group of children of varying ages including a 14 month old infant were observed watching a movie during the naptime.

A 4 month old infant was observed asleep in a restrictive d evice (swing) for an undetermined amount of time. Note: This was addressed with the caregivers during inspectoin and the infant was moved to their assigned crib.

A caregiver caring for a group of children did not have training in Pediatric first aid with rescue breathing.

A hole was observed on a wall leading to a toddler room. A door to a storage area was broken off in the outdoor playground.

Three infants were left unsupervised for an undetermined amount of time when the caregiver in charge of the group of children was observed in the office area with two other employees upon CCR s arrival to the operation.
