Based on information obtained during the investigation it was found a child left the care group they were in. The child went upstairs to a 2nd floor landing and remained there for approximately 5 minutes without knowledge of the caregiver responsible for them.

Surveillance footage shows a child being left in the cafeteria while the rest of the children and staffers conduct a lockdown drill in a separate room. Approximately six minutes go by before the child is reunited with their caregivers. The operation was unaware of this occurrence until the next day,

A caregiver responsible for a group of children was unaware that a child left the care area. The child went to a space that was enclosed and still within the school building, but was unsupervised by staff for less than 5 minutes.

Based on information obtained through investigation a caregiver did not maintain awareness of the ongoings of the children in the classroom or an ability to intervene when necessary. This led to a child inappropriately messing with another child while they slept in the classroom.

Based on information obtained through investigation it was found that a caregiver threatened a child with physical harm in response to the child's actions while in care.

Operation stated that drills were completed, but the drills were not documented appropriately.

The director of the operation has not completed the required 2 hours of transportation trainings.

At the time of the inspection it was found that an incident occurred where a child was able to open the gate and exit the playground area, towards the parking lot, when the child became upset. Caregivers were able to maintain supervision of the child and return him to the group inside the building.

Based on information obtained during the investigation it was found that a caregiver threatened to choke a child in response to a child calling them a derogatory name.

Based on information obtained in an investigation caregivers did not maintain supervision of a child when the child ran into the building, away from the operation space. Caregivers did not follow the child and the child returned to the space after 4 minutes.

At the time of the inspection children were observed going through two sets of doorways and the length of the cafeteria to use the restroom without a caregiver supervising them, or their designated caregiver notifying another caregiver the children were going through the are to the restroom to have them supervised.

At the time of the inspection children were observed talking at a volume where caregivers could only get their attention by raising their voices, spitting milk at each other with straws, crawling in food on the floor, stomping on condiment packets where they explode across the room, ignoring caregivers when the caregiver asked redirected behavior and refusing to get out from under tables when caregivers asked them to.

Based on information obtained through an investigation it was found that a caregiver did not supervise children adequately when one was dismissed from care without a parent being present and a second child left the facility with their parent without the caregiver knowing.

Based on information obtained during the investigation, it was found that a child was released from care by a caregiver prior to their parent had arriving at the operation and the child had left the building without a caregiver/parent.

At the time of the inspection 6 of the 10 children's records reviewed did not have the authorization to obtain emergency medical care.

The program did not have verification of a current fire inspection.

The program did not have verification of a current health inspection.

Four admission forms were not signed by the parent or guardian.
