Two infants were observed sleeping with a bottle in the crib. This was corrected at time of inspection when caregivers removed the bottles from the cribs.

Four bottles were observed in the infant classroom without first name and last initial at time of inspection. This was corrected at time of inspection when operation labeled the bottles with the infant names.

This was determined to be deficient as the operation did not report to CCR that a child was injured and required medical treatment.

Operation did not have a copy of operation polies which addressed each of the items required in minimum standards or comply with or exceed the minimum standards.

Two out of five children's bottles were not labeled with infants first and last initial. Corrected at inspection when operation labeled all infant bottles.

One out of ten children's updated immunization records were not available for review at time of inspection.

One caregive did not have current SIDS, Shaken Baby Brain Development training. .

Children exited the operation unsupervised through a playground gate, then through another gate and was walking outside the perimeter gate alongside of a road.

Operation failed to give parents Incident/Illness Report Form after 3 children wandered away from the child-care center unsupervised.

One infant feeding scheduled was not signed or dated by the parent. The violation was corrected at the time of inspection.

A five year old child walked out of the classroom without two staff members knowledge. The staff were not aware that the child had left the classroom.

A caregiver failed to use good judgment when she did not ask for help with the supervising toddlers while she was changing diapers.

A caregiver failed to properly supervise toddlers while she was changing diapers. As a result, multiple toddlers were able to play in the toilets.
