Operational policy did not include an emergency preparedness plan including the written plan nor the diagram.

There is no infant feeding instructions giving directives on how much and how often to feed the infant. This is required to be written and updated every thirty days with a parent signature.

The provider was using the bassinet insert for the play yard which is less than 22 inches from top of the railing to the mattress. This was removed during inspection and the mattress was removed to put at the lowest level.
