The outdoor play structure did not have any surfacing under the play structure. Note this was corrected at inspection when caregiver put tape around the opening to not allow a child to use the play structure.

The childrens product certification has not been certififed since 11/20/2022.

All three first aid kits inspected are missing tweezers and the guide.

One caregiver is missing current First Aid/ CPR certificate.

Two out of the three employee records are pending the annual trainings allergic reactions, hazardous materials, communicable diseases, and medication administration.

Director is missing the following annual trainings Emergency Preparderness, Allergic Reaction, and Hazardeous Materials.

There was an ointment that was not labeled with a child's name in the childrens changing station in the restroom. NOTE: This was corrected at inspection when caregiver removed the ointment and disposed of it in the trash can.

During nap time a child was sleeping on the incorrect mat and another child was on a mat that was not labeled with child's name.

In the infant classroom the cribs were to close to one another not giving space in between the cribs to allow the caregiver to walk in between. NOTE: This was corrected at inspection, caregiver moved the cribs giving enough space in between for caregivers to walk.

In the children's restroom there were cleaning supplies on a rack that was on the floor making them accessible to the children. NOTE: This was corrected at the inspection when caregiver placed the cleaning supplies on a door hook that made them inaccessible to the children.

A electrical outlet in the restroom near the sink did not have an electrical cover. NOTE: This was corrected at inspection when caregiver put an outlet cover on the electrical outlet.

In the kitchen area 2 deficiencies were seen. * There was a gallon of milk that had expired the date on the gallon of milk stated Jan. 24th. * In the kitchen area there were sanitizing wipes in the area where food is prepared. NOTE: This was corrected at inspection when the expired gallon of milk was disposed of in the sink. The cleaning supplies were stored away from the area where food is prepared.

The required posting were missing the up to date Daily Menu and Activity Plan. The Daily Menu that was posted was for December 19-23, 2022. The Activity Plan that was posted was for January 23-27, 2023

Written feeding instruction were not kept on file for the 3 month old infant, there were no feeding instruction available upon my request.

The emergency preparedness plan was incomplete it did not have in case of a hurricane or lock down.

Operational policies were incomplete 18 out of the 29 sections were missing.
