Paint on walls and column in the Green Room was found to be chipping and peeling off.

Cleaning supplies found in Green Room were in an unlocked cupboard and accessible to children. The caregiver in the room moved the cleaning supplies so they were no longer accessible to children during my inspection.

The operation's most recent fire inspection was conducted on 1/18/2024. The operation is overdue for a fire inspection as of this monitoring inspection conducted on 9/12/2025.

Cribs in infant room are not labeled with child's name nor is there a number labeling system.

A 20 month old child was seen sitting at the high chair table. The safety straps were not being used. The high chair table in the toddler area needs new straps that can be used to manufacturer's instructions. The straps on one of the seats has a knot tied on one side, where the strap was torn.

A child enrolled on 8/14/23 does not have a immunization record on file for review.

It was observed that one caregiver was caring for 6 children between the ages of 13-18 months old. The specified age group is 16 months old, and maximum number of children one caregiver can have is 5. The class was over ratio by one child. This was corrected during the inspection when one child was moved to another room.

The diaper changing surface in the infant room has been worn to the point of it having small rips in the plastic.

Two infants ages 4 months and 9 months old were swaddled.

The blue room was too dark during nap time. There was not sufficient lighting for eye adjustment after entering the room. This was corrected during inspection when the bathroom light was turned on.

There are no High School Diploma or GED documents available for a caregiver hired 5/2022.

The use zone on the sides of the swing structure are approximately 1 foot. The use zone requirement for swings is 6 feet on all sides with overlapping for other swings allowed.

Two caregivers did not know or have documentation available to show the ages of the children in care when they were outside.

The operation did not report COVID-19 cases within two days as required.

There was no emergency food allergy plan on file for a child with food allergies.

Based on interviews conducted. It is determined that children were allowed to have screen time for two hours a day. The maximum amount of screen time allowed is one hour per day.

I observed several bottles medication in the office not labeled with the date the medication entered the operation on it. A photo was taken.

I observed that a caregiver was not supervising children in care. The caregiver was in kitchen and not in the classroom. The caregiver did arrive back to the classroom in a few minutes.

I observed 1 file out of 6 reviewed did not have the records.

I observed a 2 month old infant in a crib with a bib and a toy in the crib with them. A photo was taken and the bib and toy was removed from the crib.
