Based on the DFPS Investigation there is information indicating that a caregiver grabbed a three year old child, threw the child to the ground and left. Another caregiver picked up the child. Note: Caregiver is no longer employed with the operation.

Based on the DFPS Investigation there is information that the operation did not notify the parent immediately about a caregiver grabbing a three year old child and throwing the child to the ground.

4 caregivers did not have the required 24 clock hours of training.

Director's Pediatric First Aid/CPR does not include the use of a CPR manikin.

Two infant feeding instructions were not signed nor dated by parents.

A staff used inappropriate discipline to redirect a child who was made to sit on a playground bench for 30 minutes.

Operation has one child with food allergies. The list of child's food allergies was not in the required postings. Corrected at inspection when the Director posted the list of the child's food allergies.

A list of Current Employees was missing in the required postings. Corrected at Inspection when the Director posted a current employees list.

The infant classroom was not free of objects that cause choking. An infant choked on a plastic piece and the heimlich maneuver had to be done.

Operation failed to notify licensing of a reportable illness within the required timeframe.

During walk through of the operation, caregiver in the 12-18 month class was out of ratio.

Children were allowed to play in the area where tar was leaking from the roof to the concrete wall. A caregiver saw children approaching the area but did not stop them causing two children to get tar on their fingers and one including the stomach area which required medical attention. Area was accessible to children and not blocked off.
