Six children's files were reviewed. Two files were missing updated immunization records.

Six employee's records were reviewed. Orientation and pre-service training certificates for one employee, were not available for review.

Sanitation inspection was conducted on 12/13/2023.

Smoke alarm was last serviced on 12/12/2022.

Posted activity plans for infant and pre-k classrooms were for the previous week. Activity plans for previous weeks did not include the outdoor actvities.

The operational policies are missing the following information: safe sleep policy for infants from birth through 12 months old; promotion of indoor and outdoor physical activity; instructions on how a parent may contact the local Licensing office, access the Texas Abuse and Neglect Hotline, and access the HHSC website; Preventing and responding to abuse and neglect of children including actions that the parent of a child who is a victim of abuse or neglect should take to obtain assistance and intervention, including procedures for reporting child abuse or neglect and Required annual training for employees; information on vaccine-preventable diseases for employees; children immunizations; and If your operation maintains and administers unassigned epinephrine auto injectors to use when a child in care has an emergency anaphylaxis reaction, policies for maintenance, administration, and disposal of unassigned epinephrine auto-injectors.

There was a crawl and climb catepillar in the outside playground on concrete. It did not have the unitary surfacing required. Note: It was corrected during inspection. Crawl and climb catepillar was removed during inspection.

Emergency evacuation and relocation diagram was missing the following: -Two exit paths from each room. -The designated location outside of the child-care center where all. caregivers and children meet to ensure everyone has exited the child - care center safely -The designated location inside the child-care center where all caregivers and children take shelter from threatening weather.
