While evaluating 4 personnel files, I observed that one caregiver that has been employed for more than 90 days does not have a CPR or First Aid Certificate.

During the inspection, I evaluated 6 children's files and the following was observed: 5 children did not have an emergency contact 5 children did not have release information 4 children did not have healthcare information 6 children did not have a special care statement 6 children did not have a healthcare provider statement 6 children did not have immunization records.

Some of the records for the six children were stored at the main head start office and were not accessible until the office could send them over.

There were two ant mounds located on the playground that is accessible to children. Photos were taaken of the mounds.

Caregiver was on her laaptop during naaptime with four children in her care.

A caregiver in charge of toddlers is lacking CPR and First aid Training.

2 out of 8 children files are lacking immunizations records. Director received emails of the children's immunizations during inspection.

A child 15 months was combined with a child that is 35 months.

Upon arrival hand washing sinks were lacking single-use disposable towels . This was corrected when director placed paper towels in the bathroom.
