Provider did not have training hours for the evaluation year of 5/22-4/23 covering the topic areas: emergency preparedness; preventing and controlling the spread of communicable diseases, including immunizations; administering medication; preventing and responding to emergencies due to food or an allergic reaction; building and physical premises safety; and handling, storing, and disposing of hazardous materials.

The provider's operational policies did not include information regarding the promotion of outdoor play.

One out of four child files evaluated was missing a health statement after being in attendance after one year. The child enrolled 5/2020.

Current immunization records were not present for review for the permit holder's two dogs.

A record of monthly emergency drill practices was not available for review. The last record of drills conducted was for the calendar year 2020.

The permit holder's two 15 year old children did not have a file for review that contained the required documentation for the inspector to review.

The operation had a total of 10 children present: 2 children (ages 5 & 12 mos), 6 children (ages 18 mos, 2, 2, 2, 2, & 3 Years) and 2 children (ages 4 & 4 years) at the operation. The maximum number of children permitted between the ages of 18 mos and 3 years is 5. The operation was over by one child. Note: A 3 year old child was sent home during the inspection.
