An infant under 12 months old was laying on a couch surrounded by blankets. The caregiver walked away leaving the infant on the couch.

The primary caregiver was unable to find the emergency drill form for review.

Observed 4 children files during this inspection. 3 children files were missing a health statements. 2 children files were missing immunization records.

Observed an infant sleeping in a pack and play with a pillow, stuffed animals and a blanket. This was corrected when the items were removed from the crib. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

There were no training records for the primary caregiver for 2021-2022 to view.

Primary caregiver did not have the 30 training clock hours required annually.

Last Fire drill was in May 2020. No sheltering or lock-down drills.
