A caregiver was observed forcing a child to sleep by grabbing the child by one arm and flipping the child over.

A caregiver was observed grabbing a child by one arm to remove the child from the restroom and classroom. A second caregiver was observed from another classroom grabbing a child by one arm.

During the inspection I observed the video and the child ran out of the room away from the class. A caregiver stated she found the child on the playground unsupervised.

A walkthrough was conducted. Three classrooms were observed and seven sinks were not working properly. Seven children sinks did not have running water. In pre-school playground, the gate was not properly secured. It is missing a gate lock. In the toddler playground, the gate was not properly secured. It is missing a gate lock. In the afterschool playground, two fence was not being properly maintained. The fence bar was seen torn on each side.
