There was one smoke detector in the kitchen that was not in working order.

The gas inspection expired in May 3, 2024.

There is enough evidence to indicate that the operation was in violation of this minimum standard. The caregiver was in the bathroom leaving the children alone in the classroom.

This operation is not in compliance with this standard. The caregiver acknowledged and video evidence shows that the child was in timeout longer than the required five minutes.

One child was missing their health statement in their files to be reviewed by CCR.

During the investigation, it was determined that a caregiver spanked children in care on their bottoms using her hand.

Evidence gathered during this investigation shows a caregiver swatted a school aged child on the hand/arm.

The van that the operation uses for transportation has an inspection/registrigation tag that expired in Jan. 2023.

There are tears in the metal underpinning on the school aged building that poses a hazard to children.

There was a caregiver working in a classroom without receiving an eligibility notification on her background check. The caregiver left during the inspection.

There are two pieces of climbing equipment on the playground that does not have appropriate surfacing. There is a piece of fencing that needs to be replaced on the playground.

The majority of staff are not current on their annual training hours.
