The children's sign-in and sing-out log was not maintained as a total of 13 children were present and only 12 children were signed in. Note: This was CORRECTED during the inspection as the director updated the children's sign-in and out log.

The director was missing annual traiing on the following topics: (1) Recognizing and preventing shaken baby syndrome and abusive head trauma; (2) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3) Understanding early childhood brain development.

3 of 4 staff records reviewed were missing the date of employment. 1 of 4 staff records reviewed was missig the follwing informaiton: (1) The Pre-Employment Affidavit for Applicants for Employment at Certain Child Care Operations (Form 2912), (2) A notarized Affidavit for Applicants for Employment with a Licensed Operation (Form 2985), (3) A statement signed and dated by the employee showing he has received a copy of the child-care center s:(A)Operational policies; and (B)Personnel policies. and (4) A statement signed and dated by the employee verifying the date the employee attended training during orientation.

The director did not have current training in Transportation Safety. The training certificate available for review had a date of completion of April 16, 2024.

Four floor mats being used by children during naptime had torn plastic and the interior foam exposed. This prevented the mats from being waterproof or washable.

A caregiver supervising a group of children was missing 16 clock hours of pre-service training.

There was no record available for review for two children in care.

The caregiver assigned to supervise a group of children in the toddler room was temporarily absent due to a restroom break resulting in leaving the children unsupervised during their naptime.

There was more than an 18 month difference when three infants were combined with older children in a toddler room during naptime. The youngest child in the group was 12 months old and the oldest child in the group was 12 years old.

Last fire drill and CO detector / fire extinguisher checks were conducted on 08/2024. Note: This was corrected at inspection when provider completed a CO detector / fire extinguisher checks and a fire drill. Checks and drill were documented under the Emergency Practices Form.

Two out of the six napping mats being used were not labeled. Two mats labeled by number did not match the assignment list posted.

Water was not available nor served during PM snack and outdoor play.

Two out of three staff files reviewed are missing notarized affidavit (Form 2985).

Food items served for lunch did not match the menu posted. Note: This was corrected at inspection when provider updated the menu.

Sanitation / Health Inspection report was not available for review at the time of inspection.

Operation does not have a carbon monoxide detector.
