Two out of six children's files reviewed are missing up-to-date immunization records.

A person in charge was not designed during director's absence.

Feeding instructions for a child not yet eating table food were not available for review at the time of inspection and instructions for another child were last reviewed on 04/04/2025.

The emergency practiices form was not maintained. Safety documentation for fire drills, fire extinguishers, smoke detectors, and carbon monoxide detector checks was not available for the months of January to May 2024.

A caregiver was missing annual training on the following topics for training year 4/21/2023-4/20/2024: (1) Emergency preparedness; (3) Administering medication, if applicable, including compliance with 746.3803 of this chapter (relating to What authorization must I obtain before administering a medication to a child in my care?); (4) Preventing and responding to emergencies due to food or an allergic reaction; (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6) Handling, storing, and disposing of hazardous materials including compliance with 746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

There was a bucket of water in the hallway which was across two classrooms used by the children. Note: The bucket of water was removed.

Two infants were not securely strapped on their chairs on elevated table. Note: The infant were strapped during the inspection.

A caregiver failed to properly supervise the children during naptime preparations which resulted in a child sustaining a forehead injury on two consecutive occasions.

A caregiver caring for a group of children in the outdoor playground did not have the key to the locked exit gate. Note: This was addressed with the caregiver and person in charge. During the inspection a different group of children were observed in the outdoor playground and the caregiver had the key to the gate with her.

The Children's products certification form used to indicate that the center has reviewed the United States Consumer Product Safety Commission (CPSC) recall list was not current. Note: This was CORRECTED during the inspection as the form was updated and posted.

There was no background check available for an employee present at the operation who had direct contact with children in care. Note: The employee left the operation during the inspection.

Nails were exposed inside a children's play house in the outdoor playground. The sharp point of the nails were along the top area inside the playhouse and posed a safety hazard for children. Note: This was CORRECTED during the insepction as the exposed nails were hammered flat into the wall of the play house.

Based on the information obtained during the investigation, it was determined that a caregiver did not demonstrate good judgment when the caregiver picked up a child by the wrists causing the child to sustain an injury.

Based on the information obtained during the investigation, it was determined that there was more than an 18 month difference when three infants were combined with older children. The oldest child in the group was 8 years old and the youngest infant was between 0-11 months of age.

Ratios were not met in the infant room when a caregiver was caring for 6 infants during naptime. The specified age of the children in the group was between 12-17 months. According to the ratio chart the maximum number of children a caregiver may supervise when the specified age of the children in the group is 12-17 months is 5. The group was out of ratio by one child.

The lighting in the infant room was not adequate to allow visual supervision of all children in the room. Note: The hallway light was turned on to allow more lighting in the room.

The diaper changing mat in the toddler room was torn.

1 of 4 children's records reviewed was missing the child's emergency contact's address, the child's immunization record, and a statement of the child s special care needs. NOTE: This was CORRECTED during the inspection as the missing information was obtained and placed in the child's file.

There were no feeding instructions available for review for 2 infants in care.

An open trash bag containing food and soiled diapers was observed on the floor next to the toilet in a children's restroom. A bottle of multi-purpose cleaner was also observed in the children's restroom within children's reach. Note: This was CORRECTED during the inspection as the trash bag and bottle of multi-purpose cleaner were removed from the children's restroom.

A hole was observed in the outdoor playground. The hole was 6 inches in depth and 1ft. 5 inches in length. Floor tiles were missing in between the door area where both infant rooms connect, posing as a tripping hazard.
