Infants were observed outside together with older children. The youngest child in the group was 6 months old. The oldest child in the group was 35 months old. There is more than an 18 month difference between the youngest and oldest child.

Operation allowed a person to be present at the operation before receiving notice from the CBCU regarding the person's eligibility.

Based on video surveillance, children were observed sitting at the table with no activity for a long period of time.

Based on information gathered, a child with special needs was not provided appropriate care consistent with their needs. Caregivers failed to plan to accommodate for the child's individual needs and attempted to force the child to do what typical peers were doing despite it being inconsistent with their abilities.

Based on video evidence, three different caregivers were observed to have grabbed a child by the arm in a rough manner. One incident resulted in the child being lifted off of the ground.

Based on video evidence, a caregiver did not relate to a child with patience when she is seen physically making a child pick books up from the floor in a rough manner.

Based on information gathered, a caregiver has been heard yelling at children in an inappropriate tone.

Three infant feeding instructions have not been updated in the past 30 days.

During the course of the DFPS investigation, it was determined that a caregiver lacked current first aid and CPR training.

One out of four personnel files evaluated lacked an employee affidavit on file.

Grass on the playground exceeded six inches in some areas.
