The caregiver that left the classroom to utilize the restroom, promptly returned and the classroom was back in ratio during nap time.

Five infants in the infant classroom were past due on infant feeding instructions requiring to be updated in the last 30 days that are consuming formula or breastmilk.

One slide on the younger children playground has screws that pose a hazard to children in care. Two screws have broken from the plastic and poses a pinch hazard, and one screw protrudes up and is a pinch hazard. Additionally, the older children playground has a metal post protruding from the ground and is a trip hazard. This was corrected at inspection with the slide removed and the metal post was pulled up and removed.

One classroom with infants and toddlers was observed during snack time and the children were no secured with the buckle in the multiple occupancy table that has buckles.

Climbing equipment on the playground that requires six inches of loose fill within the use zone had various areas that had hard and compacted dirt and there was not sufficient 6 inches of loose fill.
