A child was separated from the group of children for disciplinary reasons and in the office for over an hour.

Five children did not have the name, address, and telephone number of the child's physician or an emergency-care facility.

The operation did not have an admission record for a child in care.

Multiple infants were missing updated feeding schedules that had been signed by their parent.

There were two pieces of equipment with slides on the playground that did not have loose fill surfacing underneath them. The equipment was removed during the inspection.

Three classrooms were missing current posted activity plans for this week.

4 caregivers did not have the full 24 hours of annual training that are required.

The operation did not have their staff list, their last inspection form, or two of the required parent information posters posted.

Multiple caregivers were missing in-person CPR training that includes the use of hands-on training and practice with a manikin.

1 of 17 immunization records reviewed did not have the required doctor's record of immunizations. 1 of 17 immunization records reviewed did not have proof of the 2nd Hep A immunization.
