1 of 6 personnel records reviewed was missing the Pre-Employment Affidavit for Applicants for Employment at Certain Child Care Operations (Form 2912).

The operational policies reviewed did not include the following information: (B) Methods for increasing employee and parent awareness of issues regarding child abuse and neglect, including warning signs that a child may be a victim of abuse or neglect and factors indicating a child is at risk for abuse or neglect; (C) Methods for increasing employee and parent awareness of prevention techniques for child abuse and neglect; and (D) Strategies for coordination between the center and appropriate community organizations.

Brush was observed growing through and over a metal fence from the neighboring property in the school-aged children's outdoor playground. This posed a safety hazard as the brush was accessible to children.

A review of the Employees Who Left Employment in 2024 data from January 21, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

There was a broken tree branch at the operation.

There was no gas leak inspection conducted for the child care operation.

The vent in two classrooms was observed with dust and debris. Corrected at Inspection: The vents were cleaned during the inspection.

The child safety alarm in one transportation vehicle was not working properly.

An infant was asleep facing down in the play yard.

There was a 5- gallon bucket full of water outside the door of the Infant room in the playground area. Note: The bucket was emptied and removed during the inspection.

Director did not have a current Transportation Training.

The fire extinguisher in one of the transportation vehicles was last serviced on March 2020. One of the transportation vehicles was observed with used faced mask and candy wrappers on the floor. Corrected at Inspection: The fire extinguisher was serviced during the inspection. The trash from the transportation vehicle was removed.

An employee record did not have documentation showing how the employee meets the education requirements. Corrected at Inspection: The employee will not working as a caregiver.

Trainings hours for two employees were not available for review.

The wooden garden borders were damage exposing the nails on the connection corners. There was garden shovel accessible to the children during outdoor play. There was a bench with chipped paint at the school age playground area. There were two pieces of broken metal pole which pose a risk for the children during outdoor play. There was a wooden bridge and play structure with chipped paint at the toddler/infant outdoor play area.

The loose fill surfacing, pea gravel, for two play structures was not at the required depth.

The child safety alarm for a transportation vehicle was not working properly.
