Personnel files were evaluated for two staff members and showed 15 hours of annual training is needed.

Four staff members need pediatric first aid training.

Personnel records were evaluated for ten staff members and the following was determined: The files for four staff members did not include the hire date. The files for five staff members did not include proof of request for background checks. Eight staff files did not include documentation showing how the minimum education qualifications are met. Two staff members need a notarized affidavit. A Pre-Employment Affidavit is needed for one staff member.

The operational policies were not available for review.

Personnel records evaluated for two staff members show the staff did not complete training in prevention, recognition, and reporting of child maltreatment.

Personnel records evaluated for two staff members show the following: One staff member needs training in all health and safety topics. One staff member needs training in Preventing and controlling the spread of communicable diseases, Administering medication, Preventing and responding to emergencies due to food or an allergic reaction, Understanding building and physical premises safety, and Handling, storing, and disposing of hazardous materials.

Pre-Service training is needed for two staff members.

The operation completed one severe weather drill in 2024.

The operation completed one lock down drill in 2024.

A caregiver responsible for a group of children was unaware of the number of children responsible for. The caregiver stated ten children were present however the group consisted of eleven children. A caregiver responsible for a group of third-fifth grade children was unaware of the number of children present with the group. The caregiver stated 30 children were present then 27 children, and afterward 26. The children were counted and it was determined a total of 25 children were present.

The activity plan was reviewed during this inspection and it was determined grades 3-5 were not following the schedule.

During this inspection it was determined caregivers were not aware of the number of children responsible for. Two caregivers responsible for a group of 3rd-5th grade students were unaware of the number of children responsible for. One caregiver stated 12 children were in the group and the other stated 13 children were present. The children were counted and it was determined a total of fifteen children were in the group. A caregiver responsible for a group of 1st grade children stated a total of 14 children were present. The children were counted and it was determined the group consisted of 15 children.

A group of students were observed in the cafeteria area. A caregiver was asked who was if they were responsible for the group and replied no. It was later stated whichever staff member remains inside of the area is the responsible caregiver.

An emergency contact is needed for three children enrolled.

During this inspection a caregiver was unaware of the group responsible for. The caregiver stated fourteen children were present however ten were observed. A caregiver responsible for a group of second graders was unaware of the number of children responsible for. The caregiver stated they were responsible for seven, however it was found the caregiver was responsible for an additional three children observed in a separate area.

A food allergy emergency plan is needed for two children enrolled.

The Emergency Practices form was reviewed and showed the last lock down drill documented occurred on 12/2/2021.

Documentation of a severe weather drill was not available for review during this inspection.

The Emergency Practices form was reviewed and showed the last fire drill was conducted on 03/02/2023.

An emergency contact is needed for three children enrolled.

Upon arrival a group of children were observed on the outdoor playground unsupervised. Note: The caregiver in charge exited the cafeteria to join the group.

The operation does not have a qualified site director in place.

A group of children ages 8-10 years old did not wash their hands after returning from outdoor activities. During this inspection I observed children eating snack. The children did not wash their hands prior to eating their snack. Note: This was corrected during inspection.

A caregiver responsible for a group of children 7-10 years old was unaware of the number of children responsible for. The caregiver stated 13 children were present, however 14 were counted. A caregiver responsible for a group of children ages 4-6 years old was unaware of the number of children responsible for. The caregiver stated 11 children were present however 8 were counted.

The caregiver responsible for a group of children ages 3-7 years old was unaware of the number of children she was responsible for. The caregiver stated she was responsible for 25 children, however five of those children were being supervised by another caregiver who was apart of another group. The caregiver was also responsible for a group of 7 children ages 8-11 years old, but did not include them in the initial number of children responsible for.

The operation is not maintaining compliance and has had several repeated deficiencies. The operation has had repeated deficiencies regarding caregiver responsibilities. Several follow ups were conducted due to the operations inability to maintain compliance.

Three of six staff members present were unaware of the number of children present at the operation.

Five of six staff members present were unaware of the number of children present at the operation.

The operation did not report program and site director changes.

The operation failed to report a change in director.

The food allergy emergency plan was not completed. Note: This was corrected during inspection.

A guide to first aid and emergency care was not included in the first aid kit.

The emergency preparedness plan is missing written instructions for the following procedures: -How children will be evacuated or relocated to the designated safe area or alternate shelter, including but not limited to specific procedures for evacuating and relocating children with limited mobility or who otherwise may need assistance in an emergency; - The staff responsibility in a sheltering/lock-down emergency for the orderly movement of children to a designated location within the operation where children should gather; -Name and address of the alternate shelter away from the operation you will use as needed; and -How children in attendance at the time of the emergency will be accounted for at the designated safe area or alternate shelter; Communication, including: - The emergency telephone number that is on file with us; and how you will communicate with local authorities (such as fire, law enforcement, emergency medical services, health department), parents and us; and - How your staff will evacuate and relocate with the essential documentation including: Parent and emergency contact telephone numbers for each child in care; Authorization for emergency care for each child in care; and The child tracking system information for children in care; -How your staff will continue to care for children until each child has been released; and -How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.
