Two of the children's files did not have an alternate person other than the parent listed as the emergency contact. Note: Parents were contacted and the emergency contacts were updated to an alternate person.

There is not a hand washing sink in the diaper changing area in the toddler room. Note: A portable sink was put in the toddler room in the diaper changing area.

In the restroom in the common area has floor tiles that are cracked and broken, The light switch cover was missing. Note: The light switch cover was replaced during my inspection.

The registration sticker expired in July 2024. There was no license plate on the bus but there was a paper plate; however, it expired in 2021. Note: The registration sticker was updated when the current sticker was placed on the window.

During my walk through a pipe was observed sticking out of the wall in the infant room with a hole exposing sheet rock and inside the wall. In the school age room the paint is falling off of the ceiling.

There were bottles and spoons in the hand washing sink in the infant room. Note: A picture was taken for the file. The caregiver was informed that the sink is for hand washing only and removed the items from the sink.

Two children files out of six reviewed are missing current shot records.

The toddler room has paint chipping on the wall. The Changing mat has a rip in it causing it to not be sanitizable. Note: The changing pad was changed out for a new one during the inspection. Pictures were taken for the file.

I observed one caregiver on her phone during nap time when I walked through the room.

I observed visible dirt on the doors and door frames of the two toddler rooms. The door knobs for both the toddler rooms were loose. One toddler room had visible dirt around the changing table mat. The school age room had two holes in the wall, paint chipping, light switch was loose, and the carpet was fraying in several spots. Note: Pictures were taken of the doors, door frames, holes in the walls, carpet fraying, and paint chipping. Video was taken of the loose light switch. The doors were cleaned during my inspection.

The last fire inspection report received, via email from the director, is dated 5/7/19.

The director is unable to provide a current annual sanitation inspection.

The gas leak inspection has not been completed with in the 2 year requirement. The gas leak report received is dated for 5/6/19.

Several smoke detectors are beeping through out the operation. The beeping is a sign the batteries need to be changed.

I observed 3 bottles with an unknown liquid solution in them. The caregivers stated the liquid was bleach water. Note: Pictures were taken of the bottles and they were labeled by the caregivers.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove the operation did not have signed permission slips to take the children on a field trip.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that children did not have on any type of identifying clothing or name tag to identify the operations name and telephone number

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that there was no noticed posted of the field trip that informed parents of when, where, and what times the field trip would be conducted.

Based on the preponderance of information obtained during the investigation there is sufficient information to prove that caregivers were unaware of the minimum standards and their requirements.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that three of the vehicles used for transporting children on the field trip were not equipped with first aid kits.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that caregivers did not have on any type of easily identifiable clothing to ensure children in their group could identify them easily in a crowd.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that the vehicles used to transport children on the field trip did not have the proper safety seats for the children based on their age, height, and weight.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that the caregivers did not unload and load the children curbside or in a safe manor at the location of the field trip.

Based on new information obtained during the investigation that during the field trip on 6/8 a volunteer was left alone with a group of children to transport them to the movies.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that children were sitting two to a seat belt while being transported to and from the field trip.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that the van used for transporting children is not equipped with a child safety alarm system.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that the list used for the field trip did not have documentation that the list was being checked frequently to account for the presences of each child on the field trip.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove the operation only had 6 emergency medical forms out of 34 children who attended the field trip.

Based on the preponderance of information obtained during the investigation the director was aware of the lack of compliance with the minimum standards.

Based on the preponderance of information obtained during the investigation the permit holder is not ensuring the staff are complying with the minimum standards.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that the vehicles used to transport children during the field trip were not equipped with a fire extinguisher.

Based on the preponderance of information obtained during the investigation there is sufficient evidence to prove that children under the age of 12 were riding in the front seat of the vehicles used to transport children during the field trip.

Based on the preponderance of information obtained during the investigation caregivers who transport children, and the director, do not have transportation training.
