Out of three caregiver files reviewed, two caregivers under one year of employment did not have proof of required infant topics pre-service trainings and one caregiver employed over a year did not have proof of infant topics annual training.

Out of four caregivers' trainings reviewed during an inspection on 12/10/24, four caregivers were missing proof of required annual trainings in emergency preparedness, administering medications, food allergies, and hazardous materials during the inspection. On 2/3/25, the director submitted trainings for one caregiver, one caregiver is no longer employed, and two caregivers are still missing the required annual trainings in emergency preparedness, administering medications, food allergies, and hazardous materials.

The director was present with an inactive background check. This was corrected at inspection when a new background check was submitted during inspection and the director was observed leaving until the background check is complete. HHSC will recommend an administrative penalty as a result of this citation, per HRC Section 42.078.

The last annual fire inspection was completed on 10/19/23 which is over a year ago.

There was no current health inspection report on file for review. The most recent health inspection report was dated 01/25/2022.
