Horn's Academy Child Care Center

The operation's last documented fire drill was completed in June 2025. A fire drill was completed during the inspection to bring the deficiency into compliance.

All caregivers observed during naptime, were counted in ratio. There was not another person observed in the building when I arrived.

One caregiver was unaware of the ages of the children in her group. Another caregiver was unaware of the age of the youngest child in care and the caregiver did not have documentation of the child's age.

A caregiver was unaware of the number of children in her group.

An infant was observed in their classroom with children with more than an 18 month age gap. During the inspection, the infant was moved back to the younger infant classroom and an additional caregiver was placed in the classroom.

An infant classroom was observed to have been utilized as a passageway by the older children.

A child was observed sitting in a small highchair and was observed to not have been fastened with their seatbelt.

An infant was observed to not have been fastened while they were in a bouncer. The caregiver fastened the infant during the inspection.

A group of children were observed eating snacks out of there hands, while they were sitting on the floor.

One caregiver was observed in the doorway during nap time, supervising two classrooms. Compliance was met during the inspection when two caregivers returned from their lunch breaks.

During the current inspection, there were 2 caregivers counted in ratio during nap time and there was not another caregiver present in the building.

A 15 month old infant was observed in a classroom with children who were 3, 4 and 5 years olds.

An infant and toddlers were observed eating Doritos for a snack.

During an investigation, it was determined a caregiver hit a child in care on the arm.

A 12 month old infant was observed sleeping in his crib with a bottle. This was corrected when the Director instructed the caregiver to remove the bottle from the crib.

There were 2 infants that did not have updated infant feeding instructions. The feeding instructions were last updated on 10/3/22.

There were 3 caregivers present upon arrival, each caregiver was observed supervising a group of children during naptime. There was not another caregiver present in the building to answer the door to parents without a caregiver having to leave the classroom.

A 15 month infant was observed in a Toddler Classroom that was not regulated for Infants. Ms. Rembert removed the infant from the classroom.

Observed two caregivers not knowing how many children they had in their care. One caregiver stated they had 12 children and they only had 11. The other caregiver stated they had 9 children but had 8 children.

It was determined through the investigation only 2 classrooms have window air condition units, the remaining classrooms were observed to have been warm and not properly ventilated. During an inspection the temperature at the operation was observed to be 86 degrees.

A caregiver failed to pass on information to a child's primary caregiver when she returned from lunch, that a child in care had received an injury.

A caregiver did not use good judgment when she attempted to remove a child from a infant chair and did not move an infant in close proximity on the floor to the chair, causing the chair to hit the infant on the floor, when the chair was lifted on the ground.

It was determined through an investigation that a caregiver was neglectful in the supervision of a child in care, which resulted in the child to receive an injury.

It was determined through the investigation, parents are not informed of less serious injuries their child receives while in care.

It was determined based on evidence gathered during the investigation, the operation is not properly labeling bottle and pacifiers for infants in care.

It was determined, through an investigation, a caregiver neglected to properly supervise a child in care, which caused the child to receive an injury; due to a fall.

It was determined the operation did not notify the parent immediately of an injury the child received while in care. The parent was not notified until approximately an hour after the injury occurred.

It was determined through an investigation the operation did not report an injury, a child sustained while in care, that required medical attention.

A caregiver did not utilize the straps in an infant chair which caused a child to fall out of the chair and receive an injury.

It was determined through information obtained through the investigation, a caregiver did not utilize good judgment when they transported an infant in an infant seat outside, rather than carrying the infant outside.

Two 11 month old infants were observed grouped with 2 year children in a classroom that is not regulated for infants.

The operation allowed a person without a background submitted at the operation to transport children in care.

I observed children transported in a vehicle that had not been approved by Child Care Regulation. The transportation vehicle observed belonged to another operation.

The Director had not notified Child Care Regulation she would be utilizing a van to transport children.The Director had not completed the necessary documents for the plan of action for the month.

I observed children under 24 months using sanitizer.

Three caregivers did not have a notarized affidavit on file.

Three children in care did not have current immunization records on file.

Four children in care did not have a health statement in their file.

Two caregivers did not have a high school diploma or GED on file.

I observed a 13 month old grouped with children 3 years of age.

I observed infants sitting in a restrictive device for longer than 15 minutes. The infants were not observed to be eating.

Access to the operation was not given until 30 minutes after arrival. The inspector called each number on file and knocked and rang the bell several times before being let into the building.

A caregiver stated she had 18 children in care and it was determined she had 19 children in care.

A caregiver was observed picking up an infant, sitting in a chair on the floor, by the infant's arm.

I observed a 5 month old infant grouped in a room with 2 year olds.

A person was observed counted in ratio in a classroom and had direct access to children in care, without a background check submitted.

The children in the classroom were observed to be out of control in the classroom. The children were observed crying, climbing furnishing and throwing toys.

The caregiver in the classroom was unable to identify the ages of the children she was caring for.

A 2 year old child was observed asleep in a chair; the child was observed to be fastened in the chair with the safety straps.

A four month old infant was observed asleep in the crib with a bib on and a 10 month old infant was observed asleep in the crib with a bead pacifier clip that was attached to the pacifier.

I observed 4 children (3-4 years old) napping in a classroom with no caregiver in the classroom.

A 4 month old infant was observed sleeping in a restrictive device.

A four-month old infant was observed asleep with a propped bottle.

It was determined during an investigation inspection, the operation had 2 children in a classroom with children with more than an 18 month age gap.
