This standard was evaluated as part of an investigation and found to be deficient. Garbage was accessible to children in care.

The School Age Playground did not have the required nine inches of woodchip surfacing.

Some infant cribs did not have tight fitted sheets.

This standard was evaluated as part of an investigation and found to be deficient. Child/Caregiver Ratio was deficient in the older toddler classroom.

It was found that OV had been placed in a crib while the teacher was cleaning. OV climbed over the crib railing and hit his head.

The operation did not share with the parent the incident report the day it happened. The caregiver mentioned that she completes the incident report that day, but the other teacher did not share when the child was picked up.

Based on the interview with the parent, the Doctor's note submitted dated 1/13/2023 and the photos provided as proof stating the incident that happened on 1/11/2023 (screenshot). The parent stated during the interview that she always checks the child's body before she drops at the daycare on 1/11/2023, she knew the child does not have any bruises that day. But after the parent picked up the child in the afternoon, parent found the bruises and took a picture. The operation failed to file an incident report and did not share to the parent. During the inspection, the operation was asked of any other incident report aside from the 3 dates provided which the parent also provided the same copies. This standard was found to be deficient.

The finding was based on the photos that was sent by the parent and the statement of the former teacher who also has the same concern when her daughter was in the center, she suffered consistent diaper rashes. She would often pick her child from daycare with an extended soiled diaper. This standard was found to be deficient.

Ten children's record were evaluated and found the following: there are 5 children don't have a food allergy plan, 3 children don't have Emergency authorization, 2 children don't have health statement, 1 child has no immunization record, 1 has no emergency contact address and 1 has no transportation/field trip authorization.

Daily sign in/out were not consistent. Some children were signed in but no sign out. This was corrected during the inspection.

During the walkthrough, there were four toilets found uncleaned and not in good repair.

There were five infants with feeding instructions which are not updated or current.

Four infants were not buckled into the bucket seats.

A caregiver used an expletive when telling a child to get off of a bike. The caregiver no longer works at the center.

Licensing was not notified that the operation has remained closed since the winter storm. During the last conversation, it was communicated that they were planning to reopen on March 1, 2021. It is required to notify Licensing when an operation will be closed.
