During the investigation it was determined that the center failed to report an incident that a staff had allegedly inappropriately picked a child up in a timely manner.

During the inspection it was observed that one child was lacking their immunization and other was lacking current immunizations.

During the investigation photos were taken and the infant/toddler playground a gate leading to the parking lot had a gap that a child could fit through. On the pre-k playground the bottom of the fencing was loose that a child could get under the fence and lastly a child is able to fit between fencing and fence post.

It was observed that a infant caregiver was changing diapers directly on the rug in the classroom.

It was observed during the inspection that a child napping was placed in front of an exit door in the two year old classroom.

During the review of children's immunizations as part of the Health and Safety Audit two children of ten were lacking an immunization record.

It was observed that an infant caregiver did not wash the child or her own hands after changing a diaper.

It was observed on the counter in the school age classroom a spray bottle with what appeared to be a cleaning solution. The bottle was unmarked and in the reach of children.

It was observed that the operation was out of ratio in the infant II classroom and school age classroom.

In the gathering room it was observed that a riding toy was broken with exposed wires.

This standard was reviewed as part of an investigation and found non-compliant. It was determined that the one of the directors did not address the concerns of parents until the following week when the other director had returned from vacation.
