Chid/caregiver ratios were not met when a caregiver was caring for 10 children of varying ages during naptime. The children in the group were an infant between 12-17 months of age, five toddlers, and four preschool children.

An infant 15 months of age was observed asleep with a bottle.

A child's soiled short was hand washed and left to dry on the center's front door pull handle.

A caregiver was missing annual training on the following topics: Preventing and controlling the spread of communicable diseases, including immunizations, and Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

Director responsibilies were not met when the director was unaware that a second caregiver supervising a group of children in the toddler room had left the operation.

Five children of varying ages were left unsupervised in a toddler room during the naptime. The caregiver in charge of the group was in the cafeteria area curling the director's hair.

1 of 4 children's records reviewed was mising the child's immuniozation record.

Two employees records did not have a statement signed of the orientation received.

The employee list has not been validated every 3 months. The employee list was last validated on 6/21/24.

Director did not have a training record available for review.

A toddler was served a hot dog without being properly cut which is a choking hazard.

The sign in and out log was not maintained. One child out of four children was not signed in. Two children were not signed out the previous day. Corrected at Inspection: The times and initials were placed for those who had not been sign in/out.

The children's products certification was last done on 8/12/21.

The main door/exit door was locked.

The operation stopped carrying Liability Insurance. The insurance expired on 10/22/22. Parents have not been notified of no liability insurance in the operation.

Two personnel records did not have a copy of the CPR training on file.

The activity plan for the toddler and Preschool group was not posted. The activity plan was not dated and did not indicate for the group name. Corrected at Inspection: The activity plan was updated with the missing information and posted during the inspection.

One child's record did not a health statement and immunization record.

The health inspection was not available for review.

The child care operation did not have a current fire inspection. The last fire inspection conducted was on 09/03/21.

Personnel records were not available for review.

Two children's records were missing the following information: Date of Admission and address for the Emergency Contact Person.

Training records for one staff were not available for review.

The activity plan for the toddler and PreSchool group was not posted in the operation. Corrected at Inspection: A copy of the activity plan for each age group was posted.
