This standard was evaluated during the investigation and was found to be deficient. At the time of the incident a caregiver who was in charge of the group of children had a pending background check status. The operation had not received notification that the caregiver was eligible to be present at the operation.

I observed a child standing on a table in the Pre-toddler 2 classroom. I observed a child standing on a bookshelf in the Toddler 2 classroom. Caregivers were unaware of the child's activity in both classrooms. I asked both children to put their feet back on the floor and notified caregivers.

I observed in the Pre-Toddler Two classroom, 2 caregivers with 22 children in their care. When I asked what their specified age group was, the caregivers stated 14 months to 17 months.

I observed two classrooms not operating in compliance with minimum standards regarding group size and ratio. The director and operation owner were sitting at the front desk, and did not immediately intervene to assist in correcting the problem.

Based on the information obtained from an investigation conducted by DFPS it was determined that caregivers were not appropriately supervising the child who sustained an injury.

Three caregivers present and caring for children with pending background checks during the inspection. The operation corrected this during the inspection by sending the three caregivers home.

The files of five out of ten children's immunization records evaluated were missing immunizations according to the children's ages. The records were missing one of each of the following types of immunizations: Child 1- DTap, PCV, Child 2: PCV, MMR, Varicella, Child 3: Hep A, Child 4: DTap, Hep A, Child 5: DTap, Hep B, MMR, Varicella

Six children with doctor diagnosed food allergies did not have food allergy emergeny plans on file.

During the inspection, nine non-walking infants under 12 months of age were present with only unlabeled cribs in the infant classroom.

The operation did not report to Regulation the incident with the child that required medical attention.

Caregivers were not in close proximity to intervene when a child who is known to bite bit another child three times.
