Upon reviewing children's records, 10 children's records did not have current information regarding operational policies and water activities information.

A child was left outside in the playground without supervision for approximately 5 minutes.

A child was left in a classroom unsupervised.

I observed four cribs in the 3-12 month old classroom, with items inside the crib. Three cribs had mobiles, and another one had a toy attached to the inside of the crib. Another crib had a small, round sound machine near the infant. All of these items were removed.

An 11 month old child was able to fall out of a high chair, onto the ground. Two caregivers were present. One was changing a child's diaper. The other caregiver was sitting on the floor with her own child and her child's physical therapist. This caregiver was not supervising the children that she was responsible for.

An 11 month old child was not strapped into a high chair and was able to get out of the chair. The fell to the ground as a result.

An employee who was present and had direct contact with children did not have any of the required background checks submitted and completed.

There is not a current fire inspection available.

A child who has a diagnosed food allergy did not have the plan posted in his designated room as well as the kitchen where snacks are prepared. This was corrected at inspection.

Operation does not have a current health inspection.

Upon my walk through, I observed an outlet that was uncovered in the 6 week to 12 week classroom.

A child was left unsupervised in the classroom by herself.
