There was more than an 18 month age difference when the toddlers, preschool, school-aged, and infants were combined in the cafeteria for the afternoon snack. The oldest child in the group was 11 years old and the youngest child was 10 months old.

The first aid kits in 2 of 2 transportation vehicles were missing scissors. Note: This was corrected during inspection as a pair of scissors were obtained for both first aid kits.

A transportation vehicle designed to seat eight persons is missing the electronic child safety alarm.

Six staff trainings were reviewed, two of files are missing trainings on the following topics Shaken Baby / Safe Sleep, Hazardous Materials, Food Allergies and Communicable Diseases.

A play structure was observed with a sharp metal part that pose a risk to the children during outdoor play.

There was no fire drill conducted for the month of November 2021. The last fire drill conducted was on 10/12/21.

A monthly maintenance checklist for the outdoor play has not been conducted.

The transportation vehicle did not have a fire extinguisher.

A time sheet was not available for the director and owner/staff.

The sign in and out log was not maintained. Corrected at Inspection: The times and initials were placed for those who had not been sign in.

A children's file was missing the following Admission information: The address for Emergency Contact Names and telephone numbers of persons other than a parent to whom the child may be released Name, address, and telephone number of the child's emergency-care facility Authorization to obtain emergency medical care and to transport the child for emergency medical treatment.

The Director did not have a Transportation Safety Training.

There was no emergency evacuation and relocation diagram in the operation.

There was no activity plans for the different age groups.

The loose fill surfacing, pebbles, was on top of asphalt.

The operational policies were incomplete. The preservice curriculum was not available for review.
