A caregiver's file did not have a orientation statement verifying her orientation completion. The operation provided during the inspection, a certification document verifying the date and completion of the caregiver's orientation.

A caregiver's file did not have the completed and notarized employment affidavit. During the inspection, the operation had the caregiver's file updated, and the employment affidavit completed and notarized.

One caregivers record lacked 0.5 hours of training to complete last years annual training. This was corrected at inspection as the operation used a training from the current training year to complete last years annual training. One caregivers record lacked 4.5 hours training to complete last years annual training. This was corrected at inspection as the operation used 4.5 hours of training from the caregivers current years annual training to complete last years annual training. The director's record lacked documentation of 17 hours training to complete the required 30 hours for last years annual training. This was corrected at inspection as the operation used 17 hours of training from the current annual training year to complete last years annual training.

This standard was evaluated in regard to an investigation and a violation was found. The operation did not report positive cases of COVID to licensing within 2 days.
