The electronic fire system tag states the last test was completed 2021.

A fire inspection report was not available for review for CCR inspector at the time of inspection.

An infant was observed placed in the crib face down, on the stomach for naptime. This was corrected at inspection once the caregiver positioned the infant on the back and faced up.

During an investigation it was found that children were being kept in a room not equipped for that age group. During the unannounced inspection all children were present in rooms with proper equipment.

Based on evidence gathered it was found that a caregiver spanked a child.

It was found that a caregiver with provisions was alone with a child.

During the inspection, the fencing around the AC units on the playground have holes and are in need of repair.

In two separate classrooms, I heard caregivers indicated that the children can sleep on any cot or mat that was laid out for the children.

2 of the 6 files reviewed were missing pre-service training hours.

During the monitoring inspection it was found that there was a caregiver present at the operation with direct access to the children before their FBI fingerprint results were in.

During the investigation inspection it was observed that there was six infants ranging from 13 months - 19 months with one caregiver. This classroom was over ratio by one infant.

During the investigation inspection it was found that parents were not being given access to the operation.

The operation has failed to follow up within 7 days for 2 pending Background Check submissions. The submission for 2/15/2022 has been pending for 31 days and the submission for 2/16/2022 has been pending for 30 days.

One caregiver was present with pending background checks. This subject was the only staff present in the classroom with a group of children. *The staff was sent home until background checks have cleared.

The operation could not locate the record for one child that was being picked up by a relative. The file was not available to verify the authorization for the relative to pick up the child.

The operation has not renewed the certification for the CPSC review for 2021.

The operation has not obtained the Day Care Sanitation inspection for 2021. The last Sanitation Inspection report posted is for 6/2/2020.

The operation has failed to update the Background Check Person List as the submission for 1 subject has been pending since 10/26/2021 and a second submission for the same subject has been pending since 11/2/2021. Another subject has been pending since 10/26/2021. The list was last validated on 11/04/2021. **This was corrected at inspection when the subjects were removed from the list.

The operation did not have a current employee list posted. The lists posted did not have the name of all staff that was present on the day of inspection.

*The personnel records for one staff was not available to review during the inspection. *One staff file was incomplete and missing all of the required documentation. * The hire date was not listed for 4 staff.

The operation did not have a back-up staff or floater to cover staff breaks or cover the classroom when a staff had to be dismissed. **This was corrected at inspection when the director and front officel staff was able to take over caregiver responsibilities.

The operation has not obtained the annual fire inspection for 2021. The last fire inspection was conducted on 5/29/2020.
