Heart Dream Early Childhood Christian Academy

This standard was reviewed and found to be out of compliance through an investigation. It was determined that a staff member did not report to the parent an injury within the required time frame.

During review an individual who was let go on the 16th still had an active background check for the daycare.

During the investigation a classroom was observed out of ratio due to a group of two classes being combined with three teachers. One teacher took a group and went to her class to come into compliance.

During the inspection it was found that a group of children were allowed to pet chickens while in care at the operation. The permit holder and staff were made aware of the standard and stated that they would not have any type of chicken, duck, or reptile on the property.

During the investigation it was found that a parent was not notified by staff that their child was injured while in care.

During the investigation it was determined that caregivers did not use good judgement when they did not ask for help from the director or other staff members when children were locked in the restroom which caused the children to be locked in a dark restroom for more than 30 minutes before the doorknob was removed and the children were let out of the restroom by staff that found the caregivers trying to unlock the door.

During the investigation it was determined that a child was left on the playground for about 20 minutes alone, before being found and returned to the classroom.

During the investigation sufficient evidence was found that a child was inappropriately disciplined by a caregiver.

During inspection both infant rooms have changing mats that need to be replaced.

During inspection eight employee training records were reviewed for Health and Safety Trainings and the following trainings were not available: JG and LR need SIDS Shaken Baby and Early Brain Dev All 8 need Haz Mat LR needs CPR/1st aid JG, LR, JN, and BC need Fool Allergy training All except AB need Medication Training All except PG need Emergency Preparedness JG and LR need transportation training

During inspection 7 children's records were reviewed and 5 records were not complete and need updated.

During the course of the investigation it was learned a caregiver did not properly wash the child?s hand.

Ten staff records were reviewed and 5 staff do not have a notarized affidavit.
