The center had high school interns working here without the required training. This was corrected at inspection as those interns are no longer here and the requirements were discussed with the director.

The playground had a sharp, rusty, loose metal barrier around two pieces of playground equipment. This was corrected at inspection when the director removed the barrier and disposed of it.

3 of 4 teachers within their first 90 days of employment and without first aid training were observed with unsupervised access to children.

6 of 6 employee files did not have the preemployment affidavit available for review at the time of the inspection. The director had the employee's sign this while we were present and placed them in their files.
