Seven out of ten employees had the only online part of the required training. The staff were missing the hands-on skill practice in the CPR/Fist Aid training.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

Nine children records were reviewed. A child is missing an immunization record from their file.

Staff at the operation did not have a person in charge. The staff that assisted me at the operation were front desk staff who did not have access to conduct important task.

The operation staff did not have access to employee sign in/out sheets.

The abuse and neglect operational policy is incomplete and did not have the following information: required annual training for employees, warning signs of abuse and neglect, prevention techniques, community resources or actions that parent whose child is a victim of abuse or neglect can take.

The operation documentation on fire drills were inconsistent. There were a month skipped to three months skipped.

A caregiver was seen using prohibited discpline to a child in the ages between 3-5 years old.

An infant was seen sleeping in a restrictive device for approximately 30 minutes. The infant was removed during the inspection by the caregiver.
