There was a broken fence board near the right back corner of the playground with a gap at the ground level measuring 5 1/2 x 7 1/2 inches. There was an additional broken fence board on the playground gate with a gap measuring 7 inches.

There were two windows in the operation that were cracked and spider. One window is accessible to children and if pushed on, can possibly break out of the window frame and cause injury.

A caregiver was present and in direct supervision of children in care without a background check being entered or an eligibility letter on file. NOTE: The caregiver was sent home during the inspection.

The operational policies did not contain information regarding the promotion of active play for children enrolled in the operation.

The operational policies did not contain a policy regarding children with special needs.

The back fence of the playground is leaning inward toward the play space and needs to be secured. There is a hole in one of the fence boards that could allow a child to become entrapped.
