The infant room was out of ratio when there were five infants with one caregiver. The specified age group was 0-11 months. This was corrected at inspection when the director sent a second staff member to the infant room.

The permit renewal notice was not posted. This was corrected at inspection when the permit renewal notice was printed and posted.

A caregiver stood at the door to supervise the neighboring classroom leaving 9 infants with 1 caregiver.

A caregiver in an infant room had an inactive background check. This was corrected when the caregiver was asked to leave the operation and a background check request was submitted for the caregiver.

A caregiver in the infant room stepped out of the classroom for a few minutes leaving 6 infants with one caregiver. This was corrected when the concern was addressed with the caregivers. The caregivers will ensure that they do not step out of the room without having someone available to step in.

4 out of 4 employee records were lacking annual training in handling, storing, and disposing of hazardous materials. Additional topics that are missing include: emergency preparedness, preventing and controlling communicable diseases, administering medication, preventing and respoding to emergencies due to food or an allergic reaction, and understanding building and physical presmises.

The operation failed to submit the number of employees that have left their employment in 2023 by January 15th.

One out of six staff lacked a current CPR and first aid.

One out of eight children's files evaluated lacked a health statement sign by the physician after one year of being enrolled.

Caregiver with conditions was observed to have unsupervised access to children. CI This was corrected at inspection when an additional staff returned from break and the caregiver with conditions left the classroom.

One infant caregiver lacked current SIDS training.

Three of four caregivers' records reviewed did not have the required training hours for the last full year of employment.

One of eight children's records evaluated did not have a health statement signed by the physician on file.

The last documented fire inspection was conducted on 12/10/2021.

The following conditions placed on the operation were not in compliance: Condition 4: The director must conduct a fire drill within the first week of every month and document the date and time on a log created by the director. The log must be created within seven (7) days of the corrective action's start date. The log will also include all required inspection due dates (fire, health, gas, etc.) and safety drills. All safety drills (lockdown, severe weather) and required inspections will be scheduled/or confirmed a minimum of one month before their noted due dates. The log must be reviewed, updated, and initialed within the first week of every month. The log must be available to Child Care Licensing upon request. The operation was not following the condition that states all required inspections will be scheduled/or confirmed a minimum of one month before their noted due dates.

One caregiver was observed to be in charge of a total of 6 infants aged 2 weeks to 12 months and another classroom with one caregiver with the specified age group of 12 months was observed to be in charge of 7 children.

The operation was not following the condition that states the operation must conduct a fire drill within the first week of every month and document the date and time on a log created by the director. As of today 09/16/2021 it had not yet been conducted.

One staff file lacked a copy of a photo ID.

A caregiver was observed in thei nfant classroom in the direct care of children with a pending fingerprint check. CORRECTED AT INSPECTION: This was corrected at inspection when the caregiver was sent home.
