Two children files are missing vaccination records.

A classroom had cleaning supplies in reach of the children in the classroom. This was corrected during the inspection. The cleaning supplies were moved to the area above the stacked mats out of the reach of the children.

How children will be continued to care for the children at the relocation location until the children are reunited with parents needs to be added to emergency plan.

Criteria for when extreme weather poses a health risk to prohibit or limit outdoor play needs to be added to the policies.

Provisions for breastfeeding need to be added to the parent handbook.

It was observed during a review of video that a caregiver placed a child in a restrictive device as a form of discipline.

It was observed during review of a video that children younger than two years of age engaged in screen time on an iPad.

The most recent inspection form was not posted. It was corrected when the form was posted.

In the preschool classroom, there was no adequate light in the room. I could not see the children as I walked in the classroom. It was corrected when the night light was used.

One caregiver was caring for 6 infants, age 4 to 11 months.

The operation labeled the play yard as they post infants' name on the wall but the names were not matching with the children sleeping in the cribs.

The feeding instructions are not updated every 30day.

Five out of 6 children's records I evaluated were missing emergency contact person's address. It was corrected at the inspection when the director called the parents and filled out the information.

The operational policies do not address plan for physical/outdoor play for extreme weather.

A 4 month old infant was sleeping in the play yard with pillow, covered with blanket and staffed animal. Photos were taken.

One out of 4 staff records I evaluated was missing notarized affidavit.

The health statement was incomplete as the health care professionals information was missing. It was corrected when operation obtained information from the parents and filled the information out.

A caregiver was providing direct care to the children prior to submitting background check request. The caregiver was sent home during the inspection.

This standard was evaluated in regard to an investigation and a violation was found. The child who had possible severe illnesses (uncontrolled diarrhea) symptom was allowed to return to care before the child was free of symptoms of illness for 24 hours.
