Paint was found to be chipping away from walls in multiple classrooms including the Rompers, Toddlers, Pre-K3 and Big Kids classrooms.

Caregiver in the Pre-K3 classroom had children sit down at tables and was about to serve the morning snack when I had to stop her to have the children wash their hands before eating.

A teether was observed on the playground. A photo was taken.

The operation has failed to complete the Pre-Employment affidavit on caregiver with hire date 11/03/2023. This was corrected at inspection when the caregiver completed the form.

One child file did not have the official stamp on the immunization record.

In the romper classroom the diaper changing mat was torn. A photo was taken. The director changing the mat out.

In several classroom the walls had peeling paint.

The CPSC form has not been certified in the last twelve months. The last time it was certified was 3/16/2022.

In the Pre K 4 classroom and toddler classroom the paint was peeling, besides the cook board and behind chair. Photos were taken.

Out of six record reviewed one file did not have the permissions filled out. A photo was taken.

The notification about the field trip was not posted. A photo was taken of the calendar.

In the school age classroom when the children came in from outside they did not wash their hands.

Based on the evidence obtained during the DFPS investigation, the operation did not contacted licensing in a timely manner to report the incident.

Based on information obtained during the investigation, the licensing incident and illness report was not completed.

I observed a water hose on the play area and children had access to this area. This was corrected when the director removed the hose.

In the Pre K 3 and Pre K 4 classroom several walls had paint chips. Photos were taken.

In the toddler classroom an electric socket was missing the cover. The caregiver placed a cover on the missing outlet, corrected at inspection.

I observed a caregiver talking with other caregivers while they had children in care in their classroom. This was corrected at inspection when the director had the caregiver stay in her classroom and interact with her children.

In the school age and romper classroom the daily schedule was not being followed. This was corrected at inspection when the director had the caregivers get back on schedule.
