Several small pieces of trash were found on the out door activity space.

Loose-fill surfacing on the large playground is not installed at the six inch depth required based on the highest play surface on the operation's playground. There are also several places on the same large playground that are bare of loose-fill. Add loose-fill to attain a depth of six inches uniformly around the entire play structure surface.

Caregiver in infant room did not wash hands after changing an in fants diaper.

A three month old infant was sleeping in a swing. This was corrected when the caregiver removed the infant from the device and placed the child in a crib.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

Two caregivers were observed in the classrooms without an eligible background check. The director removed both caregivers from the classroom.

The annual report has not been done for this year. The last report was 03/22/2023.

I observed a caregiver cleaning a classroom while she had 5 children in care. The caregiver was the only caregiver in the room.

Follow Up: I observed that the children did not have cups for their water during active play only a cooler with some water in it. This was corrected at inspection when the director brought some cups outside.

In the infant classroom I observed two bowls and bottle in the hand washing sink. A photo was taken. This was corrected at inspection when the director removed the bowls and bottle.

I observed a waterhose on the sidewalk in which children had access to. Photos were taken. This was corrected when the waterhose was removed.

Caregivers with hire dates 04/30/2021 and 05/31/2022, were present in the classroom with children, did not have a background check. A volunteer was also present at the operation without a background check. The volunteer had been at the operation for 2 weeks without a background check. The director had all the caregivers leave the building.

I observed a caregiver change a child's diaper and not wash the child's hands. This was corrected at inspection when the caregiver washed the child's hands.

Caregiver with hire date 07/15/2021 did not have the required trainings for this section. This was corrected at inspection when the director completed the certifcate with the caregiver name and date the trainings were completed.

I observed on the toddler play ground that the stairs on the slide had very sharp edges. A photo was taken and the director closed the slide down until repairs could be made.

A caregiver did not know the ages of the children they had in care. This was corrected at inspection when the director helped the caregiver with the ages of the children and corrected the white board with the correct ages.

I observed a 9 month old child sleep in a child crib with a boppy under the mattress. The child did not have a safe sleep form completed. A photo was taken.

I observed motrin, tyleno, zarbee's, and other pain and fever medication not labled with the child's name or date enter operation on them. A photo was taken.

Medication was not be administered at 11 am and 3 pm per the directions on the bottle.
