Based on information gathered during the investigation, it was determined that the operation did not notify parents of less serious injuries on more than one occasion.

A spray bottle with bleach water was left on the floor near a toilet in the toddler restroom. This was corrected at inspection when it was addressed and the cleaning product was removed and stored out of reach of children.

A caregiver did not wash their hands after changing their gloves in between diapering. This was corrected when it was addressed with the caregiver.

There is no documented fire drill for the month of August 2024. This was corrected at inspection when the August fire drill was documented.

An infant room did not have working water in the handwashing sink. This was corrected at inspection when the portable sink in the infant room was refilled with water and it was verified that it was working.

The restroom in the toddler room had various items present including a chair directly behind the sink, an easel/chalkboard, there were items blocking access to the toilets, and there were various cleaning supplies in a bin on the floor near the restroom (this area was being blocked off with a chair). This was corrected at inspection when the portable sink in the toddler room was refilled with water. Extra staff was also in the process of cleaning the restroom and removing the items that do not belong in the restroom.

A caregiver changed the diaper to two children. The children did not wash their hands prior to returning to the classroom activities. This was corrected at inspection when it was addressed with the caregiver and the children's hands were washed.

The nap mats in the toddler room are labled with a number however there is no child assignment map to specify which mat is assigned to each child. This was corrected at inspection when a nap mat chart was completed and available in the classroom.

Three caregivers were unable say the ages of the youngest or oldest child in their care. The children's list they had was inaccurate.

The director's last day was May 24, 2024. Corrected at inspection: The operation's main page was updated during the inspection.

The toddler classroom did not have an activity plan posted and two other rooms had an outdated activity plan. Corrected at inspection: The activity plans were updated and posted.

The menu posted on the main board with all the required postings was outdated it was from August of 2023. Corrected at inspection: The recent menu was posted.

Ten files were reviewed. One was lacking an immunization on file and the other needed to be updated.-

Four out of ten children's files that were reviewed did not have an answer for the special needs section.

Four out of ten children's files reviewed did not have the doctor name or address under the health statement option #4.

A play table was observed full of water in the playground area accessbile to kids.

The fire inspection was last conducted on 01/30/23.

One of ten children's records evaluated did not have a health statement on file.

One infant was observed to be not strapped into a high chair during meal time. CI: This was corrected when the child was strapped into the chair.

Children were observed eating snack off a bare table. CI: This was corrected at inspection when plates were used.

An infant and two mobile infants were observed with cups and bottle that lacked the child's name.. CI: This was corrected at inspection when the cups and bottle was labeled with the child's name.

A toddler was observed walking around the classroom drinking out of a cup. CI: This was corrected at inspection when the cup was placed on the table rather than the child walking around the classroom with the cup.

Hinge-Storage box located outside on the patio is missing ventilation holes. CI-Compliance was met when they drilled holes in the sides.

One of the seat belts on the transportation vehicle was not functioning properly.
