1 of 4 children's records reviewed was missing the child's health statement.

The director and assistant caregiver had expired training in Pediatric first aid with rescue breathing and choking; and Pediatric cardiopulmonary resuscitation (CPR). The expiration date on the training certificates available for review was 4/18/2025.

The operational policies reviewed did not include information on Parent Rights. NOTE: This was CORRECTED during the inspection as a copy of Prent Rights Form 2987 was shared and added to the operational policies.

During inspection, animal droppings were observed in the playground. Note: This was corrected at inspection when provider removed the droppings.

Written lesson plans for the week were not available for review at the time of inspection.

Two staff records were reviewed and are missing 7-8 hours of annual training, including the following topics: Hazardous Materials Food Allergies Emergency Preparedness Building and Physical Premises Safety Communicable Disease

Operation does not have a carbon monoxide detector.

Inside the operation: toys and children's items are disorganized, there is a fan and chipping paint in the infant area.

Smoke Detector at the operation was not working. Note: This was corrected at inspection when provider replaced the batteries and tested the smoke detector.

Health statement for lamb at the operation is missing.

Out in the playground: grass is overgrown, animal feces were observed and there are household items and garbage accessible to children including exercise weight equipment, a shovel, empty boxes and buckets.

The outdoor play area was observed with a treadmill and exercise weights which pose a hazard for the children during outdoor play.

Two caregivers did not have the following required annual training hours: (1)Emergency preparedness; (2)Preventing and controlling the spread of communicable diseases, including immunizations ; (3)Administering medication, if applicable, (4)Preventing and responding to emergencies due to food or an allergic reaction; (5)Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic and (6)Handling, storing, and disposing of hazardous materials.

The children's product certification form was not current. Corrected at Inspection: The CPSC certification form was updated.
