The home had a free standing arch climber. This was corrected at inspection when the arch climber was removed from areas used by children.

The indoor space had a climbing play structure without the required use zone. The outdoor space had two climbing structures without the required use zones. This was corrected at inspection when the indoor and outdoor and climbing equipment were moved.

The indoor space had a climbing play structure that had components that allowed children to fall onto another part of the play structure. This was corrected at inspection when the play structure was removed.

One of the outdoor play structures had cracks in the plastic creating shear points. This was corrected at inspection when the structure was removed from the play area.

There is an above ground pool that is in part of the pathway children take to get to the outdoor space. The fence around the pool does not have a self closing gate with a lock.

Of the six children files reviewed: - Two were missing date of enrollment - Two were missing health statements - Three were missing immunization records - One was missing 1 of 4 Dtap, 1 of 4 PCV, and 2 of 2 Hep A immunizations - Two were missing doctor phone number and address - One was missing authorization for emergency care and transport, permission to transport and permission for field trips - Five were missing permission for water play which includes sprinkler and splash pad water play only - One was missing all enrollment records

There was an infant observed in a room alone in the home while the rest of the group was outside. There was a baby monitor but it was not turned on. This was corrected at inspection when the infant joined the group.

Two portable cribs were observed with covers over them to make it darker for when children are sleeping. This was corrected at inspection when the covers were removed.

There operation did not have a certified CPSC form.

The last emergency drill completed was on 5/2/25. Fire and co2 detector tests are not being documented.

There is a person living in the home without an eligible background check.
