During the monitoring visit, I was not able to view training certificates from one employees that cover the following topics: shaken baby syndrome, prevention of SIDS, understanding early childhood brain development, emergency preparedness plan, preventing the spread of communicable disease, administering medication, preventing and controlling responses to emergencies due to food allergies, understanding building and physical premises safety, and handling, storing, and disposing of hazardous materials.

During the monitoring visit, there were eight employees who completed the CPR course online. I reviewed the certificates during the monitoring visit.

During the monitoring visit, I was not able to review child maltreatment/abuse neglect certificates from three caregivers during the monitoring visit.

During the monitoring visit, one child did not have a food allergy emergency plan signed by the parent and the physician.

In the Toddler A classroom, the curriculum instructor entered the classroom and was placing a flier on the bulletin board. The curriculum instructor did leave the operation during the monitoring visit.

There were five infants sitting at the table with no straps and a caregiver. The children were sitting at a round table with chairs attached in the classroom.

Four cribs in the infant room did not have tight crib-fitting sheets.

Three out of ten children did not have up-to-date shot records available for review during the monitoring visit.

The caregiver in the toddler A classroom did not wash a child's hand after changing diapers during the monitor visit.

There was no healthcare information listed in 1 out of 10 childrens files reviewed.

There were no health statements in 2 out of 10 children files reviewed.

The operation did not have the required amount of woodchips on a toddler (6inches) and school age playground (9inches). The operation did not have loose fill surfacing under a teeter totter located on the playground (6inches).

A toilet in a restroom used by children was unable to flush and needed to be repaired. The school age playground equipment platform presented a tripping hazard and need to be repaired. The tunnel play equipment presented a pinching hazard and needed to be repaired.
