This standard was evaluated as part of an investigation and was found deficient. A caregiver inappropriately disciplined a child by grabbing a child's face in a harsh manner between her thumb and fingers while aggressively holding the child's face.

During inspection the outdoor play equipment was observed. One car had the anchor foot showing causing a tripping hazard. The rotating tricycle piece of equipment had the dirt pulled away from the track causing a tripping hazard. This was discussed with the operation.

During inspection the rooms were evaluated and there were spots on the wall in two classrooms that have sheet rock showing and breaking apart. This was discussed with the operation.
