The operation changed space by renovating and removing a wall between a classroom and office as well as adding a half wall between the entrance and pre-k room. The operation did not notify Child Care Regulation of these changes.

The operation combined two classrooms in which the youngest child and oldest child were more than 18 months apart in age. The youngest child was 15 months old and the oldest child was 4 years old. This was corrected at inspection when the 15 month old was moved to the infant room.

Three out of four play yards were not labeled for each infant. This was corrected at inspection when labels were made and added above each play yard.

A caregiver did not know the ages of the children they were supervising and they did not have a list available with the information to reference. This was corrected at inspection when a list with the children's names and birth dates was provided to the caregiver.

The sliding doors are not marked with a decal or other material at the children's eye level.

A list of current employees was not posted. This was corrected at inspection when an current list of employees was posted.

Two out four children's records evaluated were lacking a complete health statement.

One out of four children's records evaluated were lacking physician information and information for an emergency care facility.

One out of four children's records evaluated were lacking an emergency medical authorization.

An employee with a pending background check was present and providing care to children at the operation. This was corrected at inspection when the employee left the operation.
