The employee list was not posted. This was corrected at inspection when the director posted the employee list.

Out of four staff records reviewed, two did not have notarized affidavits, two did not have proof of education, and one did not have proof of orientation or receiving operational policies.

Out of four children records reviewed, one child did not have the authorization for emergency medical attention signed by the parent.

The operation did not have the most recent inspection form posted.

The operation had an expired epinephrine auto injector for a child with an allergy.

The primary caregiver did not have certificates showing training in the required special topics.

One household member was present and had access to children before receiving a background check eligibility letter. The household member left the operation during the inspection.

When submitting a background check, the operation failed to send the city and state of each residence where a person has lived outside of the state of Texas any time during the five-year period to the date the background check was submitted.

A household member was present in the home without an eligible background check. The household member left the home during the inspection.

Information obtained throughout the investigation shows that the primary caregiver does not reside at the operation as required for Licensed childcare homes per the Texas Administrative Code.

The operation did not have a Children's Product Certification Form completed. This was corrected when the director printed the form and completed it.

One child did not have a current immunization record.

2 out of 4 caregivers were missing proof of education.

2 out of 4 caregivers did not have trainings documented
