This standard was reviewed in relation to the reported allegations and found to be deficient. The caregiver pulled a child off of the shelf by the wrist and pulled the child by the arm to sit by another caregiver. This action resulted in the child receiving nurse maid's elbow.

This standard was reviewed based on the allegation and found to be deficient. An infant was given the incorrect formula.

During inspection three rooms were observed with torn nap mats. This was corrected at inspection when the operation provided new mats.

While observing a diaper change it was observed that a two year old did not have their hands washed with running water. This was discussed with the caregivers and director.

10 employee files were reviewed for the HASA and the employees had not completed the communicable disease topic for training.

This standard was reviewed in relation to the reported allegation and found to be deficient. The caregiver did not demonstrate competency when the bottle-feeding policy was not followed resulting in an infant being given the wrong bottle/formula.

This standard was reviewed in relation to the self-reported allegation and found to be deficient. The caregiver did not use good judgment when the bottle was picked up and the name was not checked before the child was fed resulting in the infant being fed the wrong bottle.

Competency was not demonstrated when a caregiver failed to properly follow the infant feeding policy that was put into place by the operation. As a result of this, an infant was mistakenly given another infant?s bottle.

This standard was reviewed and found to be deficient. The operation administered a lower dosage (inhaler) to a child whom it did not belong to.

A follow up was conducted on this day. The loose fill was measured under the swings and in front of the slides and found to be less than the required 6 inches.

During inspection the loose fill was observed on the pre-k playground. The fill under the swings was not at the proper 6 inches of depth and the fill in front of the slides on both climbing structures was less than 6 inches.

During inspection the children's lunches were observed to be placed directly on the talbe and not on a plate or napkin. This was discussed with the operation.

This standard was reviewed in relation to the self-reported allegations and found to be deficient. The caregiver did not use good competency when the bottles were not checked for names and a bottle was fed to the wrong infant.

This standard was reviewed in relation to the self-reported allegations and found to be deficient. The bottles at the operation were not labeled with the child's first name and last initial. This led to the infant being fed the wrong bottle during a feeding.
