One out of five children's records was missing the immunization record.

One caregiver's file did not have the education documentation.

There was no personnel record for a former caregiver which required an orientation, Pre-service training, education, CPR/First Aid, affidavit, and Identification.

Based on the information gathered during the investigation, it was determined that a caregiver engaged in improper treatment of a child.

4 out of 6 staff files were not available for review.

Eight staff files were reviewed. Two files are missing copy of high school diploma / ged or equivalent, one files is missing first aid / cpr training certificate, one file is missing notarzied affidavit and four files are missing certificates for annual trainings.

Cribs are next to each other and not far enough apart so that one infant may not reach into another crib.

Ten children's files were reviewed. Files are missing updated special care statement, three files are missing immunization records, one is missing emergency medical care authorization, six are missing the admission date, one is missing emergency contact name, number and address and one is missing food allergy plan or clarification indicating its not a food allergy.

Three caregiver records were reviewed. The following was not available for review for 2 of the caregivers' records: Caregiver #1 -Recognizing and preventing shaken baby syndrome and abusive head trauma. -Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS). -Understanding early childhood brain development. -Preventing and responding to emergencies due to food or an allergic reaction. -Handling, storing, and disposing of hazardous materials. Caregiver #2 -Preventing and responding to emergencies due to food or an allergic reaction. -Handling, storing, and disposing of hazardous materials.

Required records for one child present during inspection were not available for review.

Feeding instructions were not available for review for one infant present during inspection.

An electrical outlet was not covered in the infant classroom. NOTE: This was corrected during inspection. An outlet cover was installed.

Children's products annual certification was not completed. Last completed certification was 1/27/2020.

Updated immunization records were not available for review for 6 out of 10 children files reviewed.
