A seven month old infant was observed sleeping inside of a play yard with a blanket. This was corrected during the inspection when the primary caregiver removed the blanket from the play yard.

Based on information obtained there is sufficient evidence to suggest that the household member is present in the home without a background check.

A three-year old was observed sleeping inside of a blue wagon during nap time

During the walk through, their was damage to the children's classroom and kitchen area. In the classroom, their are holes in the ceiling and the wooden floor had been taken apart due to a pipe bursting. Regarding the kitchen area, the ceiling is completely gone leaving wooden beams and installation exposed and the tiles on the floor have been lifted.

The primary caregiver did not report to licensing that their was significant damage to the home that caused unsafe conditions and major repairs are needed.

Children under the age of two were observed watching television.

During the inspection I heard two smoke alarm systems making a beeping/chirping noise. The caregiver stated it has been a while sense she changed the batteries.

Of the five children's records evaluated, one child did not have their immunizations on file.
