The operation did not have a current fire inspection. The last fire inspecton was completed in March 2024.

The operational policy was missing the following sections:Promotion of indoor/outdoor play, Emergency Preparedness Plan, Nursing Mothers, Preventing Responding to Abuse/Neglect, Employee Vaccinations, Procedures for Inclusive Services to Children with Special Needs, Gang Free Zone and Parent Rights.

During the inspection, an IPAD was observed being utilized with cartoons showing, while children under 2 years old were in care. The caregiver turned off the IPAD during the inspection.

During current inspection, there was a broken toys with sharp edges observed in the playground the children were in. The caregiver removed the toy during the inspection.

A child's admission form did not have the parent's signature for the Emergency Authorization.

A caregiver informed she had 4 children in care and it was determined she had 3 children in care. The children were all grouped together during outdoor play.

The operation did not have an infant feeding instructions on file for 1 infant. During the inspection, the parent came to sign and complete the form.

4 out of 7 caregivers reviewed, did not have documentation of orientation completed.

A play yard was observed to have a hole in the play yard and was bigger than 1/4 inch.

A parent documented on their child's file that there child had an allergy to peanut butter and there was no documentation of a food allergy plan for review, in the child's file.

2 out of 7 personnel files were missing affidavit form 2985 and 5 out of 7 personnel files were missing affidavit form 2912.

During the inspection, there were 2 rocking toys and a slide on the playground that did not have loose fill around it. The caregiver removed the toys from the playground during the inspection.

3 out of 7 personnel files did not have documentation of pre-service completed prior to the caregiver being in a classroom.

2 out of 7 personnel files did not have a high school diploma on file for review.-

It was determined a caregiver had not completed pre-service prior to being in a classroom.

It was determined a caregiver had not completed orientation prior to being in a classroom alone.

A sleeping infant was observed with a bottle in the crib.

The Director has not ensured the operation is operating in compliance with the minimum standards.

During an inspection, the playpens were observed to not have snug fitting sheets.

Based on the evidence from the DFPS investigation, it was determined there was no adequate lighting during naptime.

An infant was observed asleep in a crib with a stuffed animal and a blanket. Another infant was observed in a play yard with a pillow

Based on the evidence from the DFPS investigation, a child in care received an injury that required medical attention, and the caregiver did not provide first aid.

Two caregivers were unaware of the number of children they had in their group.

Upon arriving at the daycare, I observed a television on with cartoons playing and there were children younger than 2 years old in the classroom.

I observed an infant walking around their classroom with a bottle that contained milk.

A walker was observed being utilized in the operation. The walker was removed during the inspection.

It was determined the operation did not report the supervision concern where a child was able to crawl under a fence to an unregulated area.

It was determined a caregiver informed a child they would not a cupcake, during a special occasion celebration, if they continued to cry and would get a regular snack instead.

During the inspection, I observed a caregiver leave her classroom to adjacent classroom. There was not another caregiver supervising her children. In another incident, it was determined a child was able to crawl under a fenced door and get into an area that is not regulated. The area had hazards such as cleaning supplies, tools that could have been a safety risk for the child. The child was able to be removed from the area.

The operation's first aid kit was missing a thermometer, tape, antiseptic wipes and gloves.

There was a room the children eat lunch in that did not have a smoke detector.

The operation did not test the smoke detector, carbon monoxide or inspect the fire extinguishers for the month of December 2024. The smoke detectors and carbon monoxide detectors were tested during the inspection. The fire extinguishers were observed to be within the proper fill line during the inspection.

A 13 month old infant was observed a lone in a classroom during the inspection.

The operation did not complete any fire drills for the months of November or December 2024. The operation only completed 3 shelter, severe weather and lock down drills for 2024.

The operation does not have a permit to serve hot meals and was observed heating corn and ravioli for lunch. The caregiver ordered pizza for lunch during the inspection.

The operation has not completed the Employees who Left Survey for 2024.

There were 4 toddlers observed sleeping with a sippy cup. The caregiver removed the cups during the inspection.

There were 2 caregivers that did not have orientation/pre-service on file, prior to being in the classroom.

There were 2 caregivers that did not have affidavits on file for review.

There was 1 person with a pending background check and 1 person with no background check submitted; both were observed caring for a group of children.

The outdoor area had tree branches and debris that was observed throughout the yard. A fence was observed to be broken.

A copy of the recent health inspection was not available for review.
